Please remember that you can only play 16 players in any game, so if you have extras on your team sheet they will need to be crossed off before the game commences. Failure to do this will result in a loss of points should you win as there is no way to prove that you only played 16 players.
Download copies of the 2009 Insurance Form here. This form must be signed by all players prior to playing in the 2009 season. The signed portion of the form only must then be returned to your coach / manager.
